  • Attend appointments in person or virtually (Zoom), take notes, ask clarifying questions

  • Summarize visits afterwards in writing for easy recall and record-keeping

  • Coordinate communication among all members of your care team

  • Help avoid redundant testing or gaps in care

Provider, Facility & Resource Navigation

Research and recommend qualified specialists and hospitals

  • Assist in obtaining second opinions

  • Provide a balanced discussion of risks, benefits, and alternatives

Inpatient Hospital Advocacy

  • Act as a patient liaison or surrogate family member

  • Communicate directly with physicians and treatment team

  • Monitor for safety concerns like medication errors or early discharge risks

  • Help advocate for appropriate length of stay and continuity of care

Advance Planning & Emergency Preparation

  • Assist with completion of Advance Directives and POLST forms

  • Compile and maintain a Medical Document Kit including:

    • Medication list

    • Insurance details

    • Emergency contacts

    • Advance Directive & legal forms

  • Ensure documents are shared with appropriate providers
